All services are governed by our board of Directors and overseen by The Clinical Director, Dr Kanan Pande, and The PCN Strategic Manager, Caroline Wilson.
New services are discussed at board meetings and implemented to achieve better health outcomes for the patient population and add on services for practices to support patient care. These services are part of your surgery services.

Home Visiting Team & Hub Team
Senior Paramedic Neil Boothby leads our Home Visiting and Hub Team based at Wilberforce Health Centre.
The team includes

The team helps to deliver services at the 'PCN HUB' based on 3rd Floor, Wilberforce Health Centre, 6-10 Story Street, Hull HU1 3SA.
The team also deliver home visits to housebound acute patients as well as provide proactive care and services.
At the Hub we also provide our PCN registered patients with access to:
- First Contact Physiotherapy
- Structured medication reviews
- Anticipatory care with appointments with the Health & Wellbeing Coach team to support patients with healthy lifestyle and other areas
- Social Prescribing appointments
- ECG
- Minor Surgery
Pharmacist in Primary Care
Pharmacists assist with the safe prescribing and review of medications.
As a primary care network we will also be expected to think about the wider health of our population, taking a proactive approach to managing population health, assessing the needs of their local population to identify people who would benefit from targeted, proactive support.
FCP Physiotherapy
The surgery may directly book you to see our First contact (FCP) physiotherapist who can triage your condition and if appropriate invite you in for a face to face appointment. In some cases she may need to refer you onwards for longer term support.
Social Prescribers/Link Workers
Working in conjunction with connect well and Citizens Advice Bureau. A link worker will be available to assist patients with welfare and social advice.
